Lands/Membership - Checklist for Issuing Certificate of Indian
Status for Members of Six Nations

1.     All applicants must be present.

2.     Must turn in old status card.

3.     Acceptable I.D.

  •  Birth Certificate

  •  Drivers License

  •  Passport

  •  Student Identification

  •  Employee Identification Card

  •  Provincial ID Card (With Photo)

  •  Firearms Acquisition Certificate

  •  Guarantor Form

No photocopies will be excepted as I.D.

4.     Complete Certificate of Indian Status Card Application

5.     The registered parent must sign the Status Card for the minor, 12 years or
younger. If the child is not in the care of the registered status parent, then a
legal custody order must be presented prior to the card being issued.

6.     ALL PHOTO’S MUST BE TAKEN IN THE LANDS MEMBERSHIP OFFICE
with no headgear (hat, cap) sunglasses or similar apparel worn in the picture.

7.     Lost cards must be reported to the Lands Membership office, there is a 30 day
waiting period which commences on the date of reporting to the office.

8.     There is a fee per card.

9.   CARDS ARE ISSUED THURSDAY & FRIDAY ONLY,
FROM 8:30 A.M. to 3:30 P.M.
