JOB SUMMARY: Reports to and works under the direction and supervision of the Clinical Lead RN and the Team Manager of the Diabetes Wellness Program and Six Nations Elected Council which includes matters pertaining to the policies and procedures of Six Nations Elected Council. To provide supportive and collaborative chronic disease management services to those living with or at risk of developing diabetes. These services can be one-on-one or in a group setting to help community members achieve well-being.

Wage: Minimum $70,400/year
